The work you should not have to do yourself

Care coordination is our team working behind the scenes so that every part of your treatment connects — medical care, counseling, insurance and community resources. You are dealing with enough.

What is included

  • Coordination between all members of your treatment team
  • Referrals to community resources as needed
  • Communication with primary care providers, with your consent
  • Assistance with insurance and access to care
  • Ongoing case management throughout your treatment

Why this matters more than it sounds

People rarely drop out of treatment because the treatment failed. They drop out because a prior authorization stalled, or a referral never got made, or nobody told the prescriber what the counselor was seeing, or transport fell through on the wrong week.

Care coordination exists to catch those things. It is unglamorous and it is often the difference between a plan that works and one that quietly collapses.

Insurance and prior authorization

West Virginia Medicaid requires prior authorization for most substance use disorder services. That means a request has to be submitted, medical necessity documented, and a decision received before treatment begins.

You do not submit that paperwork. We complete it as part of intake and tell you where it stands. If coverage lapses or a claim is denied, we work the problem rather than handing it back to you.

Working with your other providers

If you have a primary care physician, a psychiatrist, a probation officer or a case worker, treatment goes better when those people are not operating blind. With your written consent, we coordinate. Without it, we do not — you control who knows what.

Beyond clinical care

Recovery runs on stable ground: housing, food, transport, employment, legal matters. Where those are shaky we refer to community resources rather than pretending clinical treatment alone will hold.
